Disable GemInstaller install in default rails preinitializer.rb

It breaks stuff like Passenger, and you really shouldn't need it anyway (should install manually in dev, and via capistrano/chef on deploy, and via CI pre-build hook in other environments)
Also added (commented) fork support for people who do still want to use it, thanks to Gabriel Gironda (http://github.com/gabrielg)
